The Fusion Youth Group is ecstatic to give our teens the opportunity to fellowship with each other in a Christian environment. There is no doubt that our activities, outings, and trips give us just what we need to fuse together and encourage spiritual growth while having fun!

However, in order to keep our activities going and our kids safe, each participating teen must provide us with a Fellowship Baptist Parental Consent Form before taking part in any activity. This includes any and all visitors you will be bringing with you.

All you have to do is PRINT, SIGN, and RETURN the Fellowship Baptist Parental Consent Form prior to, or on the date of, a scheduled activity or outing. It's as easy as believing that Superman is afraid of Chuck Norris!
